I find recognising issues that the learner may be experiencing in class as early as possible, avoiding the problem to escalate, is always the best, whenever possible.
The most useful strategy that has worked for me is to have several 1:1 discussions with the student to find out what has worked for them in the passed. Following on from this I would share with the teaching team and implement. Providing visual & written information that is reiterated several times in small parts.
